Below are all of the required forms for TLC 2009. Please register using the pdf or Excel form provided. Registrations can be given to Tammy Bouwmeester or Robynne Wylie. For out of town players, please contact Robynne or Tammy by email (on the Contacts page) for mailing addresses. We would prefer cash for the $20 registration fee, but cheques can be made payable to “Truly Lacrosse Cares”.
